She has a medical condition that requires therapy every couple of months. Streaming (aka sitting in chair for 6 hours straight every day) is like the opposite of a therapy. Her family has performed an intervention, and forced her to prioritize health first. The thing is, the optimal thing to do would be to stop streaming entirely and dedicate all this time to therapy, which would considerably increase her chances at getting better in the long run. However, Gura does enjoy streaming, so she made a deal where she will be allowed to have short windows of streaming activity in between therapy periods, as long as it doesn't interfere with therapy too much.

It has been revealed in membership videos and posts, and from the other members - Gura had a lot of health problems last year including fainting, to the point she pretty much had to have staff or another member with her whenever she went anywhere or did anything outside of her hotel room. Even with her health problems, she was still flying back and forth to Japan to do BTS work and record projects. She even told members in advance she likely would not be streaming for most of if not all of last summer because she had so much BTS work. The other girls even mentioned they only saw Gura sleeping in her room or working in the studio this last time they went to Japan together due to being so busy. When she did finally finish all her work she took time off to sort out her health problems and personal life. Because all the streaming she did before the hiatus and all the BTS work she did during it left her burnt out.

Unfortunately her return just so happened to be right before everyone was getting ready for Holofes, so now she’s gone again until that’s all sorted. Once Holofes is over she should be back to streaming on a somewhat more regular schedule again. But the reality is that Gura will never be back to the 12 hours long streams of the first year. Is not only her health that won't let her, but Gura is now way more than a streamer, and the pandemic is over. She is an idol now, and she is the face of the branch, so she will always be involved in a lot of BTS things and big projects, that especially for Cover, will always have more priority than her streaming.

Anon be more suspicious and there was an unusual number of HoloEN talents that suddenly needed "health" breaks in 2022 that were easily doing 7 hour streams in 2021. There was something going on with contract negotiations that escalated so much that Ina had 5 months worth of breaks and "lost" her work phone for 3 months. Gura, Sana, and other talents were getting into fights with management and Cover did replace the EN project manager.

You are right about her having health issues, and you don't even need to be a member to know that because she has talked about it on regular streams multiple times. However, it is cope to say that she's not streaming because of those health issues. Except for that earring story, every other time she talked about fainting was related to movement like getting out of bed quick or whatever else not related to sitting. If you're a person who faints easily, what's the best job you could do? A sitting job. Gura streamed 2020-2021 regularly without fainting a single time on stream.
There are other issues she talked about like fingers/hands going white, which is probably related to Raynaud's syndrome or some other condition, but that also is not an excuse not to stream.
In fact, she clearly stated that she unplugs from the internet and spends time doing random shit, and that she has "small battery" aka she's too lazy to do many things. If she does some recordings or any other holo-related activity, you can be almost guaranteed she won't stream on that day or for half a week. Also, in one of her "return" streams where she played MC 2-3 times in a row, she has mentioned her friends, so she's not some sort of shut-in girl with no people to hang out with like some anons would have you believe.

tr;dr
She has problems, but they're not preventing her from streaming. She's just enjoying her life to the fullest by spending her time how she wants with whom she wants, and streaming is an afterthought.
