I don't have the exact formula to give you but I can say a couple things I believe are key to bettel's success.
1- Obviously you need a somewhat attractive model, otherwise it'll be a lot harder.
2- Self deprecation is key, it's how you prime your audience to put you down and humiliate you and make fun of you.
3- Be funny, this is likely what will make the difference between a cute loser and an offputting one. Without this, number 2 will just lead you down into being menhera. Being funny about your self deprecation will help people feel like they can bully you and make fun of you without feeling guilty, since it'll just be a bit.
Also Bettel has already gotten hospitalized multiple times for mental breakdowns, so maybe that helps the keyfabe.
