Practice speaking slowly, enunciate properly, use pauses instead of filler words and focus on one topic at a time. Your goal shouldn't be to speak perfect RP English, but to be as clear and intelligible as possible with the accent you've got. As long as you're intelligible, your unique accent might even become a strength. xPetu and Bausffs, just to take some examples from League youtube, got really strong accents, but it works well for them. I'm also an ESL who tends to mumble and fumble in casual conversations, but after practicing these techniques I've become a pretty strong public speaker.
