i think in general small content creators just enjoy having people watch them.
even if they're there out of pity/sympathy it's better then having nobody and it gives you a chance to make them a real fan.
despite what people here said you don't really think about the identity of your audience when you stream/make videos, it's not like you can see them, they're sorta just one person in your head.

Yes they are creepy. It's OK to say nice things, like to Aqua: "I hope she feels better". This kind of comment is general with a general positive direction.

The creepy aspect aims towards their direct nature. A concern fag to Aqua would be like: "Oh you should get some rest", or "Sleep now, it's not healthy for you." There's a very "control" aspect to this. Gratification would be linked if said streamer actually follows such commands.

Saviorfags and concernfags are to different types of people:
A saviorfag is the end result of numberfagging. They only really care about the vtuber's performance. As such they will stick to a vtuber that they aren't a fan off because the vtuber has low numbers, and immediately drop them when those numbers become too high.
A concernfag is already watching the vtuber for the vtuber. They are too invested in the wellbeing of the vtuber, to the point where it may feel like they believe that they have a "real" relationship with them. Concernfags do not care about numbers, unless the vtuber shows some signs that their numbers bother them.
Let me put it like this to make mybpoint clearer. Aki, who is known to be a rather well adjusted vtuber, has saviorfags. Korone, who known to have a large viewership, has concernfags.
