Look up videos on youtube about English native speakers talking about their experience about working in Japan's English teaching program.
Basically their textbooks were written by someone who barely understood English more than 4 decades ago and every Japanese English teacher learned English from said textbooks and barely speak the language.
Also the native English speakers can't correct the Japanese English teacher because it's a social taboo in Japan and they just end up repeating the shitty incorrect and bad lesson materials.

You forget to mention that the Japanese government keeps it like this by design to protect their entertainment industry. I remember a thread on /v/ posted by a Japanese anon who claimed he worked for a marketing company specialised in electronic entertainment. Same, two semesters of spanish with someone who routinely swapped Mexican Spanish and Castilian out while she spoke and didn't know the difference. Her standard of speaking spanish was that the Janitor, who was Cuban, nodded along while she spoke it to him.
