Because that's the sort of thing that would happen, and has probably happened often enough. His characters tend to have a lot of 'mask off' moments that reflect their inner human. He has literally recorded himself having conversations with girls on phone dating services.
I think for this scenes it was inspired by how he got the actress to portray her disgust by describing the scenario and then asking her how she would feel, and she said "disgusting" and he said that's it. The actress said he was a pretty weird guy, too. You have to understand the broader context though, with otaku no video, and the fan reactions and death threats for the Evangelion tv show. It makes perfect sense why he put this scene and other scenes in.
