Not vtuber but related, I went to Japan to see magical mirai 2017 after being a miku fan since about 2008, it was a huge deal for me. Anyway the short story is I got hemoroids and my ass was sore as fuck the entire trip. Come concert day it was still uncomfortable but I pushed through it and cheered and jumped and waved those fuckin sticks for 2 hours non stop.
When the lights came back on and we went to leave, I found that my butt didn't ache anymore and my pants were drenched in blood. Turns out the roids burst from all the jumping I guess. It was like I sat in a bucket of paint. Anyway, the concert was epic, would do it again in a heart beat.

I specifically ate light with protein/snack bars on the day of the concerts for this exact reason. I did need to pee before Stage 2 and the toilet lines were awful, but if you get there early enough you have more than enough time. Just prep in advance and/or go before getting there. If you have the capacity to plan out your trip to Japan to see your oshi, you can plan out a day to avoid shitting your pants at the worst possible moment.
