You should Never get a full image from someone the first time you commission them, especially if they ask you to pay first. Ask them for a sketch, then if that's acceptable, pay to upgrade it to a full pic.
If they've taken your money and you haven't heard back, message them. If they don't respond at all, ask them publicly on one of their posts so they feel forced to reply (this is only if it's been a month or so without any response).

Constant creativity repeatedly is difficult, especially when you need to be creative in a way the client likes. If you add on the idea it's a character you don't really care about, it just becomes draining after a while.
