I remember hearing her speak Italian once, and she really had a weird accent (pretty thick and northern), to the point that there were other Italian dudes in the chat pointing it out. Also, I asked her why she didn't stream more in Italian in chat sometime ago, and she just answered that it would be pretty hard to get an audience if she did so, so I'd guess that going for the EN and ES audiences while keeping her image of an Italian girl doing so is just smarter for overall viewership.
