I remember last year part of her health problems was her doctor telling her to cool it with using her voice cuz it was seriously straining her or something, but obviously she couldn't do that. My theory is that's finally reached a point she can't really ignore it, or at least she's decided to confront it to not have it be a massive problem in the future, and so she's gotta keep streaming to a minimum, which explains the vague but long timeframe.
As for why she won't say why, maybe it's serious enough that in a worst case scenario she wont be able to stream anymore and she doesn't want us worrying about that, or something like that.
