It does, at least insofar as that they are less fit than other applicants. The issue with nepotism is that favoritism is shown to a person who should not rise to that position in a free market if judged by their own merits. "Unfit" in this case means lacking in relevant skills, or having a skill level that's insufficient to produce the desired result. Both the business and customers get fucked over by it in the end.

Also, consider the following. Suppose similar resumes hit your inbox, and a reliable employee tells you that he knows one of them and can vouch for their skill and character. Are you not allowed to prefer that person, or to not even allowed to hire them? Trustworthiness is a factor in who you hire, but one that's difficult to gauge long-term by interviews. Personally, I wouldn't hire him because he's an employee's friend, but I would consider that trust in the equation, because "the best person for the job" isn't just about their skill level, it's also about whether they'll steal or lie. Is that still nepotism?
