I think uploading content is just one factor that helps with the algorithm. I'm not familiar with Bill Wurtz, but I imagine the rarity of his videos helps hype up his existing audience when he does upload. So he gets tons of views from his existing audience, who also probably help spread the word that he's uploaded which results in even more views, which then translates into the video getting recommended by the Youtube algorithm.

Kiara (and holos in general, I imagine) rely on both regular uploads and views to help get them into people's recommendations. So while the Let's Play you're subscribed to makes daily content, they don't have the views to boost them in the eyes of the algorithm. Whereas Kiara uploads some stuff every other day while on vacation, but gets hundreds of thousands of views.

And to be honest I kinda understand her hospital/doctor anxiety. I'll say this much: I was hesitant to go to the doctor too. I had back problems a while, and then, I had a problem with some food I ate and had to be rushed to the hospital. It turned out one of my nonessential organs was bad. Once it was removed, my back pain immediately went away. Turns out that organ had been messing me up a lot more than we realized.
If I had just gotten it checked out earlier, I wouldn't have had to go through all that pain.
