How has there never been a major incident of a vtuber's app/program accidentally revealing their real face instead of their avatar? Do these programs really have that many built in fail safes?

Yeah, the apps themselves are obsessively safe. It's way more common for the chuuba to fuck up her OBS, accidentally pressing the camera capture instead of screen capture button. The modern apps for turning face capture to vtuber movement have several failsafes, though it is still a possibility.

Information is sent from one thing to the next thing. Why would you be sending full face information to the app that renders for the stream? Only a few apps did that because they were coded by absolute amateurs. Basically all you should be sending to control your character is a handful of dots/vertices.

That only ever happened to the one chinese dude vtubing as female but there have been multiple instances of accidental face reveals from reflective surfaces on handcam streams zombie girl. There's no software failsafe that will save you from that.

Modern streaming software generally has features that by default doesn't capture/transmit video feed while capturing the data for your face and body's expression and position parameters
