Anime face is not as off-reality as people think, it's at least way more reality based than cartoon style. There are just a ton of details that a lower skill person might not recognize due to the softened features in anime. But things like cheekbone, chin protrusion, brow ridge, eye socket, laugh lines, and even the infamous anime nose are all features you can recognize when you study real face. There are some jp and korean artists that breaks this down in their art courses. The key is that anime softens a lot of the features, and often hides edges and depicts the shapes as shadow/light shapes instead. A big example is how a lot of "serious" mood anime art renders the Rembrandt triangle. That wouldn't happen if anime truly forsake realism. The matter of their 3D models is quite different due to how 3D rendering engine works. Even with cel shade lighting model too much details will cause too complicated of a shadow shape on their faces, which will ruin the style so they have to soften the features even more, getting it even more deformed.

Can someone help me understand what I'm doing wrong? I thought it was canvas size but the lineart on the left is half the amount of pixels I use for mine (1170x1170 compared to 2000x2000), so why does it look so much cleaner? Is it to do with drawing with a digital pen, or what? It's been quite frustrating.

But it’s not cleaner? It’s better but it’s definitely a sketch which is part of why it looks better: lineart looking better than the sketch is a tale as old as drawing. I’d say your Mio lacks a little line weight variation and the lines are a bit stiff, and the proportions are a little off compared to the left particularly in the head and face, right breast, and her foreshortened left leg.

you've fudged around lines to more inaccurate positions to a degree where face's proportions start to feel a bit too off. Sketch leaves more gap between eyes for example. Chin is centered more to the right in more correct position. Eyes are positioned more straight. Left leg is more towards viewer in perspective and shape is leaner.

Sketches in general leave a bit more to imagination because lines are more vague and not settled and you get some leeway from that. When you try to consolidate the broad sketchy edge to single line you might settle to a wrong detail that then looks more wrong. This happens less as you understand the 3d shapes of the anatomy and perspectives more.

The sketch on the left is from someone I asked for help with as to why my drawing looked bad. So I redid my drawing based on that. I wonder if I can fix this.
