They're either an adaptation of existing fanart or a 2 min thing the talents do in Photoshop overlaying a few avatar poses they have over some background of the game or the game title.

They mostly use a lot of fanart. When someone uses their tags on twitter it's basically a tacit agreement/request for the Holo to use their art in a thumbnail.

It depends. Some holos make their thumbnails from scratch. Most holos make their thumbnails using fanart. In big collabs the host makes the thumbnails for everyone. Then you have Towa who hired a dude to make them for her because it was too annoying lol.

Hololive does not interfere with thumbnails. The talents do them all themselves. Talens usually use a lot of fan artwork (always with consent) to make the thumbnail.
Artist love providing their art and some are super dedicated: Watame has blueta sensei under her spell. The guy posts a high quality piece on twitter for many of her streams just for fun. But he takes commissions too, he worked stuff for Noel and the sheep many times.
His most well known work are the illustrations on Watame's video letter to Coco.
