The facts:

In a way yes. The generalists you see are basically the t1 tech support of doctors. Their only purpose is to throw out antibiotics and general care. If you give them the right keywords and they aren't burnt out from being on the bottom rung of doctors and seeing thousands of patients with vague symptoms they'll refer you to the specialists.
Even then those guys aren't perfect. If you told them you ate moth balls they'd easily be able to identify Naphthalene. However not many doctors will have the knowledge that termites produce naphthalene. Even if they were told that she lives in a goddamn termite cave (which she probably didn't) they'd be quick to just go for the celiac since it's the more common DX.

I would normally have said that the termite toxins wouldn't really be a thing because tons of people have had those and don't get hugely sick from them, but here it's an exceptionally bad case for Ame as the levels are probably concentrated in a smaller, presumably not very ventilated apartment (especially in winter right now, all the windows are gonna be closed) and the nest ventilates right onto her as she sleeps so... god damn, fucking yeesh. Filtering it won't do any good either, it's too small to be affected by those. The chemical also only lasts a few hours in the air, like not even half a day. This would also explain why it's getting worse now in wintertime. I don't follow Ame closely enough but if she was also having adverse enough tummy horts during the height of summer heat, aka a time when all windows are gonna be closed so she can run the AC? That'd also be evidence to suggest it's a buildup effect from a massive ongoing infestation.

So the immediate thing Ame should do is open her windows and have as big a fan as she can stand the noise of blowing the air out to get rid of it. However yes being winter time that probably would fucking suck, I wouldn't expect her to do it for long periods but definitely at least briefly open the windows and try to blow it all out for an hour a few times a day. As we're heading into spring Ame should have more opportunity to just pop open the windows and leave them open all day which would help.

A few years ago I had this issue where I would regularly have extreme pain for multiple days in my right ankle to the point that I had a very noticeable limp or had to use forearm crutches.
In the worst case i was completely immobilized and the pain was generally worse at night so I also had a couple sleepless nights because the pain kept me awake.
Obviously I asked multiple doctors about it but nobody could give me a diagnosis. Usually they'd prescribe me painkillers.
One orthopedist took an X-Ray of my foot but didn't notice anything unusual.

After over two years of dealing with this shit a relative suggested that I'd get myself checked for gout and lo and behold it was gout. The uric acid levels in my blood were way too fucking high.
And knowing that I could finally take measures to avoid these fucking gout flares. First with medication and later with dietary changes, i.e. less meat.

Did you ever have a blood test in those two years? I think uric acid is one of the things a standard blood test is designed to look at.
But yeah, the primary issue with doctors primary issue is that they see thousands of patients with the sniffles so if you have something slightly rare he's going to handwave it.
Gout is rare enough outside of the Mediterranean a Western doctor isn't going to see it unless he observes the uric acid level or you tell him about a heavy organ based diet.

Ankle presentation is definitely just enough off of the usual for gout that some doctors would be absolute garbage at diagnosing and would default to trauma or general inflammation. If you'd presented with toe pain the med school pattern recognition would kick in.
