There have been several cases where viewers are bled because of m*les being mentioned too often and many of the chuubas (Peko, Marine, Coco herself) take it pretty seriously since they don't want to deal with both antis and any fanbase repurcussions.
But again, yes, this mostly doesn't apply to EN. Most of the viewers are normalfags who don't care, so outside of antis being vocal and cancerous like always, if Gura got dicked live on stream it probably wouldn't significantly hurt her numbers or growth longterm.
