If she was off her meds it would be much worse than bugs. People worry about her hallucinating because it could be a sign her meds are becoming less effective, which could lead to her mental deteoriation. It's highly likely that's not the case since she shut it down as being pretty normal to happen to her from time to time, but it's one of the more logical sources of brainworm we have.

I have a confession to make, if the movie Shonshon picks sounds mid I'm standing her up.

Shondo wanting to have her body mutilated is clearly rooted in a desire to escape her mental cage for a while, to shock her brain with pain and maybe connect with You physically since she can't really do it another way.
