Impossible to say. Without having access to Cover's contracts, we can't be certain what equipment (if any) is provided for specific talents, or what talents do and don't have to pay for. Even the cost of a design/rig isn't certain, because Cover is being charged enterprise rates so they could be charged way above (or way below) market prices.
However, they're almost certainly profitable. There's basically zero upkeep cost post-debut, since managers serve multiple talents and even multiple gens. Even being a 3view is enough for many people to afford streaming as a full-time job.
