You ask this question in the same thread where dragoons exist.
At a certain point you stop caring about their physical appearance.
Because psychologically speaking the character is greater than the person behind them.

It's only when you can separate chuuba from operator that you can really ask yourself that question; AND even then if they are really attractive like Reimu, who's absolutely cute and beautiful IRL you even need to remind yourself that the liver is just a normal person at the end of the day.

Basically most fans wouldn't give a shit, normal people and antis would find it hilarious and mock them if they were ugly though.
