That could definitely happen with the right artist or design changes. I think the mangaka of Miss Kobayashi's Dragon Maid did a really good job with character design since there's a clear separation between Kobayashi, a flat businesswoman in her late 20s, and the actual lolis in the show. With flat VTubers it can be a lot harder to tell the difference.
