Look at Veibae for ex. She's a good looking girl (irl) with a decent personality. So it's not that she doesn't has friends, she's just staying in front of pc and interacts with her viewers.
Mouse once said something about parasocial relationships. People always think it's only that viewers that have them, but many of the streamers are having pretend relationships with their audience as well.
Look at Reckful. He was surrounded by thousands of people and always alone. His real life friends were always busy, and his audience was just not real. So he killed himself (of course depression is also to blame).
Mouse said when she started streaming she did it because she was looney of being always at home alone.

Veibae in RL should be careful not to disconnect from real people too much.
The problem is, online every time she goes live thousands are waiting for her and loving her. In real life she is just another person, not a star.
So you rather stay online.
That can be dangerous.
