Somebody suggested a while ago using regional prompting with vertical rows and latent couple if you plan on using loras. Use lora 1 for the top 1/3rd of the image to get the head then use the second lora in the rest of 2/3rds of the image to get the costume.
Also make sure you are using the right loras. That image was done with SDXL and the XL lora. It won't work on 1.5/EF models.
