It's a great coping tool. Not for everyone but it works for me.

If you spend a lot of time in waves of agony the down time from it turns into bliss. I hate pain but the feeling when it's gone is the some of the best you can ever feel. I talk a lot about Ike chocking me but honestly this really should stay a fantasy. Asphyxiation is really, really painful but intimate if it's hands-on and is known to help with pleasure if not overdone. So it's a perfect fantasy scenario that should be kept there.
The emphasis on coldness that you noticed is interesting to me too, because I tend to prefer cold and distant things to warm and close ones in almost all areas of my life. Both metaphorically and literally.
