New outfits don't strictly have to be from their mama or papa, but you also presumably don't just get them for free whenever you ask. I figure if you have the means and creativity to commission a lot of outfits then you'll get a lot of them, like how Rin for example clearly has a lot of costume ideas and can afford to make them reality while someone else might not really have any particular ideas for themselves.
Creating a new one from scratch can be a fair amount of effort in general, Ange showed off a bunch of drafts from her latest outfit in her zatsudan yesterday, and they put in a lot time with Kawaguchi-mama hammering out the final design and details.
