Some fish like picrel are very, very small. It requires no suspension of disbelief to think yours might be some tiny enough to swim up someone's urethra. I do think your current ideas are a bit more /ringo/ than /meat/, especially as there's no justification for why the fish developed this behavior. Why do they infest humans specifically, and why lay eggs in the bladder? I think this idea works best if the eggs must be laid in the bladder because it's an ideal hatching environment for them. When the itty bitty fish hatch, they then begin eating away at the inner lining of the bladder. When the host urinates, the hatchlings latch on to the host's inner walls using those aforementioned barbed teeth to prevent being swept out. If not dealt with, the fish will eventually eat their way out of the host for a messy, painful death. If you want it to be especially daunting, give the species extreme gender dimorphism. Females are small enough to swim up a human's urethra but males grow significantly larger, causing all sorts of complications for the host as the brood begins to mature. This feels more /meat/ to me.
