There are many models. There are some that are double-edged and other that are just dildos that sit on your groin and doesn't really do much. Most of the pleasure is just psychological as the guy gets emasculated and the woman gets to feel in charge. It's frankly a lot of work since the guy is always way bigger and heavier so he needs to do a lot of compromises in terms of positioning, cleaning things out beforehand so you don't have a brown mess, and dealing with the fact the woman probably can't really thrust that well. if there is no extreme psychological pleasure then its just not that good.
