Depends on the severity I would say, if it has the general shape right then (before upscale) I send it to img2img set de-noise to 0.4-0.5 and re-roll a few times to see if It gets it right.
If it's really severe sadly inpainting is the only way but again do it before you upscale, should make it easier.
