The penalty for true death isn't the story ending (since that would fuck up a lot and be kinda 'WHAT'), it's losing part of who you are to a mysterious entity for the opportunity to wind back the clock to the critical moment of failure and change your destiny. But things will never be the same regardless as the world around you will grow darker and change; or someone else close to you may be the one to take the blow instead. Changes happen subtly at first, but it becomes incredibly obvious that things aren't the same if it keeps happening, especially with regards those around you.
I want losing in battle to a severe degree to have real consequences without just 'lol the story is over now ty for reading goodbye!' since in any RPG where that happened the players would lynch the GM. Severity of said consequences is dependent on who or what is being fought.
