Wildcards do save their "rolled output" to the metadata by default. If it only saves which wildcard you added, then it likely didn't work at all. I know at least from my experience that if I see __Headwear__ instead of (Hat:1.2) in the metadata, that's because it failed to initialize the wildcard (usually because it isn't installed or was misspelled).
For the second part.
If it's all on one row, then that's just one roll in your wildcard which will add them all at the same time.
Each row is a different "roll" on the wildcard.
If you want multiple separate rolls within a wildcard, you need to add wildcards to your wildcards.
Example:
You want clothing, so you make __Shirt__ and __Pants__
__Shirt__ contains three lines, T-shirt, Tank_top, and Hoodie
__Pants__ contains two lines, Jeans and Skirt
Now you make a new wildcard called __Fashion__
__Fashion__ contains one lines that goes __Shirt__, __Pants__

Later you can then add __Jackets__ with some stuff in there, and then make __Fashion__ Look like:
__Shirt__, __Pants__
__Jacket__, __Pants__

And then it will either give you a shirt+pant combo or a jacket+pant combo
