The progression of the disease is due to progressive decline of dopamine in the substantia nigra part (literally black substance, because dopamine stained black with the first used stains) of the brainstem. Dopamine is a neurotransmitter and part of its duties are to regulate movement. So its a functional disease regarding the motor system not a structural one. Motoneurons being directly affected? Thats minimal.
