But right after we learn Death-sensei had friends, we learn "I had a sensei too". Which means he wasn't the first Grim Reaper, but an apprentice of the previous Grim Reaper who still might not have been the first one.

On that same note, I'm unconvinced that "first apprentice" is chronological - It could just as easily mean that she's first in rank & responsibility, a designation that she is "first" in line to inherit the position of Grim Reaper once the current one is replaced.
And maybe that's the culmination of her training, in a reflection of her real-life angst of overcoming her role models:
Maybe the First Apprentice's last mission is always the same: to defeat Death itself, take his soul for themselves, and take on his duties until the day she raises up a reaper capable of killing even her.
