Mario 3 had a bunch of random shit that rarely appears anywhere. Like the fire hammer bros. You'd think they'd be a regular enemy, but they only appear twice in the game, both in places that are pretty easy to miss.

Because that's partly what made it cool, it was a little thing that made the level stand out. Occasional, novel experiences are what help make the game memorable. That said, yeah, there's a good chance it was meant for more than one level at some point in development, but for whatever reason the other levels which featured it were cut or had the shoe removed for making the level too easy or generally not geling with the layout.
