AI is pretty same-samey for the entire game, and then you meet brick wall 8-3. Absolutely unreasonable, and then you make it to the final "staircase". I was barely able to do it with save states, and it took me at least an hour. I don't know anyone is expected to do it on their own on an actual cart... Up until World 10 the levels are very hard, but world 11, aka the developer levels are... just.. mind-fuckingly hard.
The game even gives you save states once you reach the dev levels. It's like they knew you're gonna get fucked. Some guy on nicovideo beat only 5 levels of the last world, but that was about a year ago. Dunno if he beat it eventually.

Try beating Chips Challenge without looking up the solutions for the levels. There's so much fake difficulty with invisible walls and trial and error (some blocks contain instant death fire under them and there's no way to know beforehand) that it can get downright sadistic.

I can't give you an objective answer for hardest, but among the most difficult I've played are...

Adventure Island
Journey to Silius
Ghosts and Goblins
Tower of Druaga (early curve before getting the boots and one or two other items can be torture)
Abadox (FUCK THAT SECOND LEVEL)
Challenger (on a higher difficulty)
Final Fantasy (slow as fuck, enemies that can wipe out your party from time to time if their deadly magic hits, etc)
R.C. Pro Am II (good luck trying to get 1st every time!)
Battletoads (fucking duh)
Kid Icarus (pinpoint precision platforming. Even if you get health upgrades from grinding like crazy for points, if you die once you restart with only one section of your life filled of your max)

And those are just NES/Famicom titles. I'm sure there are plenty more that are fucking nerve-wracking.

I don't know, Little Nemo wasn't that hard. Especially for a Capcom game.

The Train was annoying, sure. Also that house-level.

And the last one was pretty mean, especially because you had to replay that stupid forest and it's boss every time you got a game over.

But it was more than doable, and the Nightmare King was easy for the last bossu.

If this version counts, it definetly is among them. I can beat both rounds on NES without dying once, but the arcade version will kick my ass any day.

Sure, it's all easy and shit at first. Then after you start the post game (yep, an arcade racer with a fucking post game) it starts raping your anus. Just 'cause it can.

Often times, you have to have an absolutely perfect run, or you'll lose. and That's on the first set of "extra" stages. It only gets harder after that. And don't get me started on those goddamned car attack stages...

On top of this, AI cars don't perform the same as they would in a player's hands. They all get better and better as the game progresses. So you can't rely on just driving a "better" car to help you win most of the time.

And even unlocking the secret cars is insane at times. Like one where you have to get a perfect score on a Galaga minigame, and one where you have to down a fucking helicopter with your car...

I know some people don't see the "challenge" in racing games, but play this one (or F-Zero X and F-Zero GX for that matter) and you fucking will. I guarantee it.

I don't know how I missed this one the first time around.

Gauntlet for the NES. It has to be the NES version. The NES version is special, since it has 100 set levels, and actually does have an ending.

In order to beat the NES version of Gauntlet, you need to access EVERY secret level in order to get every part of the vault code you need to access the final level. If you miss one, you will be forced to guess what letter/number it was, and if you get it wrong you will game over instantly as soon as the final level starts. You can't save, you can't put in more credits for more health, and many of the levels towards the end of the game are full of exits that will lead you in level loops, back to earlier levels, or back to the start of the level you were just on, all the while your health is ticking down.

I've seen the ending and the last 21 or so levels with the help of a password with a known vault code and some cheats, but I doubt ANYONE has ever beaten the NES version of Gauntlet legitimately. I'd be impressed if anyone managed to make it halfway through the game without cheating.

Legacy of the Wizard. It is pretty cool, but most of it is way more linear than it looks. Most of the challenge comes from finding the items and using them in puzzles.

To compare, twelve year old me was able to beat this game without making a map but never beat Back to the Future 2&3 despite trying to map it out.

The fact that BttF was boring as watching paint spontaneously generate probably led me to give up sooner though.

I don't know. The hitbox is really difficult to deal with-- in general I think that the sprite of the surfer needs to be shrunken down. I had an easier time playing Gradius,for example, because the hitbox and the game mechanics in general were so refined that whenever you died you felt like it was your own fault, not that it was a cruel game.

It's the difference between screaming "Damnit!" at yourself in frustration, and "fuck you!" in frustration to the tv

Yeah, but good luck getting every single secret room. That and mapping out the maze while health is constantly on the decline. You will be playing from the start hundreds if not thousands of times trying to get it right.

I've also heard it's even worse for a full party of 4 since you have to share the power-ups that you need for late game survival, but I've never been able to get three people together who were interested in playing through all 100 levels of Gauntlet with me.

Just having to do it all in one sitting(since passwords don't save keys, potions, or power-ups) is enough challenge alone. Granted, you could save state your way through, but I still don't think many people if anyone has ever managed to beat this game legit.
