Marathon: Infinity did it best. There's a singular invisible mini-S'pht which does not move but shoots projectiles at triple speed, and on hard difficulty it's a real pain to encounter when you're not expecting it. The enemy seems completely arbitrary, as if the devs just felt like creating a quick turret enemy without creating new assets, but if you explore the full level, you'll discover that a brilliant overly-ambitious Pfhor commander was demoted and sent to a remote outpost in the middle of a giant swamp. To pass the time said commander engaged in a number of experiments on creating new kinds of weapons to help the Pfhor, and in his hidden laboratory is an area with S'pht being experimented on in crusher-rooms, explaining the aberrant one encountered earlier.

For whatever reason, Unreal Return to Na Pali has these blue skinned Skaarj (not to be confused with the blue Skaarj Berserker towards the end of the main game) who only appear in the snow level on easy, being replaced with Mercenaries on higher difficulties.

No this was a bridge disaster back in 2006. Everyone who is on the bridge in the pic, except the man and woman on the right, died a horrible death. They fell more than 100 feet while screaming all the way down. Their bodies couldn't be retrieved for weeks due to weather, which led to vultures picking them apart. Random body parts or scraps of clothing would appear on people's lawn because vultures tend to be messy eaters in the sky. The entire park was closed for a while, and reopened only recently. Legend has it that at night you can hear children wailing and whimpering, asking for their mommy. Spirits who died a sudden death are often left confused about what happened, making them unable to pass on. An extremely sad state of affairs really.

You fight the enemies there multiple times, but I like how the Black Omen in Chrono Trigger has some new and bizarre looking enemies. There's only 4 of these guys in the game for example, and they don't respawn.
