Pretty sure half the iconic designs by Roger Dean (and a few others) for Psygnosis were just the product of "oh man, that's a cool sci fi picture you've drawn there, slap a heavy metal looking logo on it and we can use it as box art".

To be fair though, the metal endoskeleton design was reused for the floaters

>oh wow this game looks awesome I must buy it!
>this guy never appeared
I was devastated at the time, but the bait paid off in the end.

The most intriguing ones aren't the ones that have nothing to do with the games, they're the ones that take everything directly from the games EXCEPT only one element is nowhere to be found in the game.

Pic related, no underwater stage.

This game is a perfect example, the artwork by Richard Clifton was used for the cover of a Blue Oyster Cult album from 1980, then for Ork in 1991

Doom has that SMG-like gun on the cover and those demons that don't quite look like anything in the game.

the cover from Powerslave also has everything straight from the game, except there is no shotgun in the game.

Featuring Cody, Damnd, Guy, and that green guy peaking out from the manhole cover (that's also on the SOR1 box).
All of which are nowhere to be found in Final Fight 2

Golden Cheep Cheep on the SMB3 box art isn't found anywhere in the game. In fact, there's a whole level on the back not found anywhere.

Years later, both were discovered to be in two unfinished levels, I believe the unknown level is a 7-4 beta level and the cheep cheep is in one level dubbed "Atlantis" by players.
