Probably Silver Surfer but the strange thing is that I beat it as a kid and there were plenty of easier games that I could never beat as a kid, but because they were less straightforward, whereas Silver Surfer just required persistence.

For me it's Warriors of Might and Magic.
I wonder if most of the difficulty came from it being bootleg, wonky hitboxes and controls and all, but I could never beat the intro level. Or maybe I was just too much of a casual.
Anyway, I gave it to my friend as a "bonus" when I sold my GBA.

As an avid NES fan, I've beaten many of the games people tend to tout as being very difficult. Battletoads, Castlevania 1, Double Dragon 3, Ninja Gaiden 3, TMNT1, Solomon's Key. But I always want to default to Ghosts n Goblins because of how unfair it feels, even in spite of how much more shit the other games have given me.

If I to be pedantic, than I'd say one of the never ending games like Pacman or Tetris is the hardest though.
