The design mindset was that coming out of arcades where you were forced to play at a cretin level if you wanted to continue. Everyone who played had to reach said plateau, but thats also part of the reward in that everyone may get there in different ways but you're all have a shared expereance. Its not the case if one person plays on "easy" and "beats' the game, its not comparable to the experience of someone who played on the normal/hard difficulty.
With many older games, you need to treat "easy" as more of a "practice" mode then anything.

It sucks when the game isn't properly designed around it. Hard Mode in DMC changes enemy placement such as putting Shadows, Fetishes, and Death Scissors in more places and making them more aggressive, but in Borderlands 2 it just makes enemies have more HP which spirals into insanity due to the exponential scaling.

Blasphemous did it best, its NG+ is balanced around being maxed out from normal mode, and you get to choose one of three gimmicks to mix things up such as replacing the life bar with individual life orbs, as well as adding new content.

Easy mode is never gonna give you all of it anyway. At the very least on most games you're gonna be missing certain enemy types on certain areas, certain areas are gonna be way more empty, bosses won't use all attacks and so on. So even if you could play all stages you would be missing out on content.

I play many games on easy; some of them let you enjoy the entire experience regardless. Others just cut the ending, but the game per se is intact. Then, there's a few that just say "lol fuck you" and you do have to get better, if you can and want it.

I feel like a lot of early Sega Genesis titles did this, usually having the easiest mode be called "Practice" with less levels, the best examples I can think of from the top of my head are Castle of Illusion and Spider-Man vs. The Kingpin, though I'm sure there are plenty more that I just haven't gotten to yet, which reminds me that I need to get through my backlog more instead of just replaying what I already like, either way there are other examples outside SEGA.

I didn't have too much of a problem beating both of them on "Normal", and eventually I even did it on "Hard/Nightmare" even, but I do dislike this system because some people prefer to play on easier settings and I think cutting the run off early because of that is a shame, though I still think it isn't too bad because usually the default medium difficulty settings are manageable with enough practice, which is kind of the point of retro video games anyways.

What I really dislike is when the later levels and/or ending are locked behind the hardest difficulty setting, because then you're essentially asking the player to have absolutely mastered every level just for a chance to see the ending, which again I don't have that much of a problem doing because I try beating everything I play on the hardest setting eventually anyway, but I know not everyone can do that, for a variety of reasons, so it doesn't feel right to me.
