That's almost certainly a dosbox problem. If you are willing to put in the time and effort, try PCem.

I did get this to run smooth at one point in DOSBox, wouldn't recommend it unless you're a purist and if you're playing the whole game through in one sitting without saving since saves fuck up the difficulty. Some other problems would be music not looping, input lag - which shouldn't matter if you're playing with autoaim and mouse aim off - and enemies randomly taking damage at the start of a level leading them to patrolling when they shouldn't be otherwise. You'd probably hear a death scream at the start of Phantom Express when a cultist crouch strafes off the train. Otherwise, check out this guide, I recall using vesa_nolfb at one point to prevent the hud skipping. https://steamcommunity.com/sharedfiles/filedetails/?id=286998267

Sourceport, or play Blood through the /vr/ dospack, it has the bMouse fix, plays MIDI and no stuttering (atleast for me)

This is what happens when the hardware can't keep up with what DOSBox is trying to do. You'd think that an i5 should be good enough but it might have some features like self-throttling to reduce what the CPU considers inefficient usage. Our hardware is programmed (via firmware usually) to "think for itself" more and more often.
I have a Phenom II and never could get DOSBox to work well with high fixed cycles, but something like 40k or maybe 50k worked for some games. You'll have to tinker with it a bit to get it to work right.
Alternatively you can get a cheap laptop to run something like XP on for dedicated /vr/ time. You probably won't be able to get 98 installed on bare hardware due to lack of drivers for it nowadays.

Literally unsolvable in DOSBox and trust me I tried. Play GDX or similar.

Blood has some issues in DosBox which aren't easily solved, in spite of what DosBox cultists may insist, which is why people clamored for a sourceport, and were mighty disappointed that nuAtari left the official rerelease unfinished.

You still have stuff like the damage timer on the choking hands being tied to your clock speed, meaning they will auto kill you.
