OP is referring to MAME 2003+, which is a fork of MAME 0.78(which was released back in 2003). It's a "core" for retroarch made for low power devices(Rasp Pi shit emuboxes) using a lot of the older and hacky drivers for various arcade systems. This was done because some people believe that MAME added in a bunch of "unneeded" stuff like slot machines back in 0.79, and therefor 0.78 is considered by them to be the last "clean" version of MAME. Some effort was made in back-porting some drivers and games and fixes from mainline MAME, but it's still very much outdated. You also need to put together a special snowflake romset for it since the mainline 0.78 romset won't work with it, or download the specific 2003+ romset.

To put it bluntly, there is zero reason to use MAME 2003+ unless you are trying to play shit on a Rasp Pi or similar toaster piece of crap. Any PC/Laptop from the last 10 years will run normal MAME which will run more games and have less bugs and emulation glitches. Anyone who doesn't want the slot machines doesn't have to download them.
