There's an entire power rail that goes unused, the 12 volt, if you don't have a GDROM drive in it
This makes the regulator responsible for that rail run a bit toasty. It's fairly easy to mod it out if you have any soldering experience at all.
Them AliExpress chinks are even manufacturing new power supplies without this problem too.

As this anon >>7282907 mentioned, the DreamCast will run hotter with a GDEMU because the 12V rail (normally used for powering the drive) goes unused. As the DreamCast uses an old linear PSU, all the power for the 12V will be produced whether it's needed or not, and all of it has to go somewhere, so it is dissipated from the PSU as heat.
However, for ~$50, you can perform the PicoPSU (I think that's what it's called) mod, which replaces the old linear PSU with a modern switching PSU. This way the DreamCast will stay much cooler, and it's a much more elegant solution than installing a stronger fan. Plus it removes a bunch of electrolytic capacitors that are part of the original PSU, which would probably eventually require servicing anyway.
I did a GDEMU+PicoPSU install on my DreamCast and I'm very happy with it.

It's hard to say for certain. I don't think it will cause immediate damage, but every part has a finite lifespan, and running them hotter than they're meant to will certainly shorten that lifespan.

If you have an ODE you can still experience the drive noises as the developers originally intended by installing the Authentic Sound Perfect Immersive Experience mod. This connects to the ODE and recreates the drive noises through a small on board speaker.

Don’t worry about it. GDemu creator says the heat is nothing to worry about. He doesn’t even recommend you remove the 12v rail. Personally I removed the rail and the resistor and my Dreamcast has been fine. Heat is obviously bad for components but there definitely hasn’t been widespread reports of dying Dreamcasts as a result of GDemu heat. Just remove the rail + resistor (though I’ve also heard removing the resistor isn’t necessary, mine hasn’t had any problems without it) and don’t worry about it.

I love this shit because it makes it cheaper for me to buy Japanese consoles and just replace the PSU to support the 200-volt span instead of having to gut a more expensive PAL console for it. Most earlier consoles like the Super Famicom just used a generic external power supply with a barrel connector.

Well, next time, I would suggest you get a DreamPSU. I would replace my internal PSUs even if I lived in Japan because the modern ones are simply better, they don't produce as much heat which is apparently very useful with the Dreamcast especially, they don't pull as much electricity, and some of those old PSUs might start blowing capacitors soon.

Why is this? Aren't all the individual power outputs specced for standard voltages like 9, 5 and whatnot? And a lot of OEM power supplies were notoriously unreliable, like early PS1 revisions.

Because the bulk of the PSU isn't even in the case anymore, you hook up a power brick like this instead of basically having the equivalent inside the Dreamcast. That, coupled with the Dreamcast PSUs being prone to failure over time (black screen reset) and reducing heat inside the console are compelling reasons.
