Why did this game, specifically, have so many distinct clones? Rambo and TMNT come foremost to mind as ones where it was obvious. Did Nintendo break character and actually put out the source code and let other people use its engine for official releases?

Companies just copied whatever Nintendo did and was popular at the time. Look at the countless number of Zelda 1 and Mario 3 clones.

They borrow the formula but normally those games have a lot of their own quirks. Rambo and TMNT on the other hand play like they're fan-hacks of Zelda 2.

TMNT has its own quirks though, 4 characters, no experience system, no magic etc. I don't know enough about the Rambo game to say otherwise.
