It really doesn't. It's just a combination of the fact that the defender gets a constant influx of prestige in most scenarios, and the AI is too stupid to use it effectively, so what it constantly does is spam infantry or other cheap units when it notices you getting near a city.

Seriously, go play Barbarossa, play both sides, and just keep hitting end turn. The Soviets gain 120 prestige a turn. At Kiev, it's 160. Lots of scenarios have them, especially ones where the Russians are on the defense.
