If you don't press start the game takes you to a demo of 1st level which reacts only to start button, making you feel disoriented like if the game started but the controls are unresponsive. There had to be a "Press start button" line to actually tell you how to begin playing.

It's a documented bug in the title routine that was probably left there due to rushing and was never addressed again, probably not to be embarrassingly apparent to people with different revisions, or maybe they didn't care I dunno. The instructions that are supposed to print out to the PRESS START BUTTON tiles and make them flash use the same RAM location as the ones used for the SONIC TEAM PRESENTS text, which was probably a very late introduction (earlier protos probably just used something similar to the hidden credits screen, which as >>5568081 mentions, does help properly display the PRESS START BUTTON text; apparently the SONIC TEAM PRESENTS still gets loaded in that case, but also gets killed right away).

Since what was there in those earlier betas was likely another piece of text that used the background layer as opposed to active sprites, the game didn't have to clean up the SONIC TEAM PRESENTS code, so when it loaded the PRESS START BUTTON code it didn't run out of memory. In the final game it does, so the code is never truly loaded and thus the text is never shown.
Apparently the code for that text is shared with the one responsible for the trademark logo beside Sonic (and we kinda know it was put there at the last minute due to a very late proto featured in an ad), so it may be possible that by including it, it fucked some calculations in the dev's brain which thought it only had to clear enough RAM for that one TM logo or some shit, nobody knows for sure. I'd check how the code works but I really don't want to right now.

I'm more fond of ROM hacks that make the special stage rotation smoother (it's choppy to make it exactly line up with the tiles, but it's so choppy that I never really thought it was worth the trade off).

just see some big 10MB rom with literally just fucking everything, streaming audio from cartridge (more than doable back in the day, it just chews through your ROM space, Sonic VR is a nice example -- there's four PCM songs, shit's like 6MB), every fucking character in the series, dozens and dozens of custom abilities, etc, etc

It'd be a mess, but it'd be a beautiful one.
hell, for the PCM audio, you could reasonably chunk bits of it up and have an order list that it jumps around through instead of streaming it as one big song
