Honestly? Environmental design and attention to detail. There are battles that moved through corridors until they reached dead ends that you can trace by the severed limbs, bullet holes, broken guns and shredded bodies.

At one point you even find a formerly barricaded cafeteria full of corpses of unarmed people who were gunned down by cyborgs. Severed limbs and items aren't normally dropped at random in System Shock 1. Skulls are a little more random, but it's not like Doom where they're just like "Let's throw some tortured people's bodies here and there, killed marines to be spooky". The important part is it's not something that stands out and is obvious like Bioshock 1's environmental design, it's something you have to pay attention to and notice, and it adds to the atmosphere.

The level designers made the station first and THEN added robots, traps, and where people died, and as a result it has a subtler, more important sense of place.

It's crazy to think that both these games came out in the same year. System Shock 1 was amazing for it's time in 1994. It does use a fully polygon 3D engine (unlike Wolf 3D and many of it's clones). It was an evolution on Ultima Underground game engine. The controls for the original game were a bit weird, and it played like a mix between a FPS and a point and click adventure with some minor RPG elements and an inventory system. The game has an incredible sense of atmosphere with horror elements (this game created the audio log trope), HUB based locations with non-linear level design. The game is like an FPS Metroid. There wasn't anything else like it for PC in 1994. The MIDI soundtrack is fantastic too and so is the ambient audio folly. The only issue with the original System Shock was the controls. But there are mouse look mods and the Night Dive Studio port that fixes this.

Both System Sock 1 and 2 are classics. The game can be speed run through in about 10 minutes, while System Shock 2 can take about 15 minutes. The System Shock series were the pre-cursor to the BioShock series. But I've always liked SS2 the most. I think System Shock 1 stands up on it's own against Super Metroid.

Yeah, the controls take some getting used to. All movement has a keyboard binding, you don't need to click to turn and stuff. You can alt+click to double-click or automatically loot things rather than dragging and dropping.

Assuming you're talking about the original controls. Mouselook makes it "easier" but it also has its own problems and it's not really a game designed for it.
