Japan puts Hitler in games all the time. He's just a historical "bad guy" that's kind of humorous to put in weird scenarios. It's not like in the west when they do it solely for the sake of pushing political propaganda in their games.

It really isn't once you get the hang of it. In fact as you get into the game you can stock up on a bunch of continues. The rocket launcher destroys everything and the only real difficulty in the game is the bionic arm platforming which is great since that is the purpose of the game. You do need to make the perfect shot on Hitler at the end though.

Nintendo of America didn't finalize and enforce their censorship policies in time for this. Some other games around that time ('88-'89) got away with language that would get other games rejected.
