We enjoy the ride while it lasts and the full engagement with demanding game mechanics. Pilotwings Expert and F-Zero on master mode can give you a challenge. Fire Emblem 5 (aka Thracia 776) might be one of the most challenging /vr/ strategy RPGs in existence. Puyo Puyo is one of the hardest puzzle games to get into (seriously, any other /vr/ puzzle game is more accessible). Haven't played many shoot'em ups yet but I guess that Axelay and Summer Festival Recca 92 are a few I games I can think of. Megaman 1 is probably one of the hardest platformers on the NES.

The hardest to 1 loop have been mentioned already: Tatsujin Oh, Same! Same! Same! 1P Ver., and Gradius III Arcade are definitely up there
