They didn't, that's just how CRT's work. All developers knew this and took it into account while making their sprites and backgrounds. When CPS1 games are at 1:1 pixel ratio they made everything on screen too fat, so that when it gets squished inwards by the CRT the final result is it ends up looking correct.

yeah CPS1+2 use a widescreen resolution that gets squished to 4:3, meaning the pixels aren't square. It looks weird as fuck on a modern monitor with sharp pixels. Adding scanlines helps to break up the tall pixels a bit.
It must've been a nightmare to develop for, I wonder if the artists still painted the graphics on a machine with square pixels?
It gets even weirder with some games like D&D:Tower of Doom, you can see some of the in-game art was clearly designed for the squish (like the moon in the background of the forest scene, which was drawn wide so when it got squished it becomes a perfect circle) but then some elements aren't designed for the squish. Most of the UI looks designed for square pixels, like the little diamonds on the HUD that look too tall when squished, or any character portrait that appears (they're perfect squares if you use square pixels).

I remembered playing a game in the arcade that was run & gun where you were on Dolphins. It was 2 player simultaneous play, one as a guy the other a girl. I thought it was fun but tough, maybe because I was not familiar with it, but sadly the arcade around me replaced it with Guilty Gear probably because aside from myself and my cousin, no one played that machine. I don't know if it ever got any home conversions.
