Maybe the romhack for Super Mario 64, Super Mario 74. FFVI Brave New World was fairly challenging for a JRPG as well. Beat Adventures of Lolo 1 and 2 a few weeks ago, those are pretty hard without a guide. Also it's not /vr/ but I did get 100% achievements on Rogue Legacy, which yea, most of it isn't that impressive because you can technically grind for it but I did do the optional challenge bosses which force you into set builds.

I rarely played arcade games growing up, so for me it was mostly NES games and then the harder Genesis games like Ecco.

Also not retro but the DS Castlevania games with hard mode lvl 1 cap is probably the toughest since those days. I'm more into multiplayer games for challenge nowadays.

Off the top of my head I'm not sure. irl people are impressed I beat Contra without the code and only died twice on the last level. I grew up with Kid Icarus as well so I actually find that game to be pretty easy. It's hard to say because normies will be amazed that I can pretty consistently beat Earthworm Jim 1 & 2 but on this forum it's probably not a big deal.

I've also beat some tough modern games. Wipeout HD's platinum was brutal. Mega Man 10's Mr Perfect was fairly tough as well. Ecco definitely wasn't easy.
Last level in particular was exceptionally hard and if you lost the boss fight (which you're likely to do given that the vortex queen can instakill you by eating you and you're forced to go within the range where you're vulnerable to that move if you want to do damage to the jaw since it's invulnerable to the sonar attack) you'd have to go through the whole level again.

Cleared the entire classic Mega Man Series with no e-tanks and just the uncharged buster outside of forced usage of utilities/weapons (I.e. you have to use the magnet beam in MM1 in the room between the footholders and yellow devil, bubble lead on alien wily in MM2, etc.)

I don’t play too many already “hard” games, I prefer doing challenge runs on Normal games like Mega Man and such. Mike Tyson’s Punch-Out is the only truly hard game I’ve beaten, and I still think beating wily in Mega Man 7 without e-tanks or special weapons was harder than punch-out

Ecco is a pretty awesome underwater Metroid-like. Good production values (especially in Ecco 2 which is gorgeous) and reasonable difficulty except for a few scrolling levels (Welcome to The Machine, Tube of Medusa). It's even got multiple endings and shit. Just don't bother with Junior and Defender of the Future, those are shovelware made by unrelated people.

Hmm, maybe Rondo of Blood? I didn't thibk it was THAT hard thanks to the save/password system, but it was definitely challenging.

I find it easier to play games that are hard, but you can break into chunks like that rather than something like Shinobi 3 where it's back to the start when you die. I find that game incredibly difficult by comparison.

Bloodlines is another Castlevania that's tough, but really good that I finished. I did end up using passwords with lives instead of my own to beat later levels, so doesn't really count.

God Hand doesn't count here because it's too new, but that game is one of my favourite hard games I've beaten

Dark Souls is my favorite game of all time, but on raw difficulty it's not much harder than the hard NES games, especially since you can increase soul level to make things easier on yourself. I'm not sure there's anything that compares to Ornstein and Smough on the NES, but tbqh that fight can be annoyingly cheap. Gwyn is hard, but not any harder than Mike Tyson from Punch-Out.

I thought Battletoads was a very hard game, but after some times playing it I can pretty much beat it easily, since the game is based around memorization and some parts are hard because of the instant death stuff.

Personally, I would say anything I've 1CCed in the Gradius series. Shoot-'em-ups require a lot of practice and autism. I've yet to finish Gradius III arcade or Gradius V. The gaming 'tism has started to wane as I've gotten older.

In the third one it's all about tapping the jump button often and only really pressing it for long wide jumps, the recognizance of the jump button is mushy in this game but if you get that part down the challenges are way easier.

Also the hitboxes are weird but easily exploitable to beat the challenge bosses too, focus more on down and sword to attack vs forward and sword. The patterns are there if you study them too.
