you have to run withouti changing directions longer than you'd think. it basically tricks the fireballs into missing you. and you just have to take potshots witht he sword when you can get it. can't really rely on items here.

Personally though OP, I gave up after like a fourth attempt at beating this game, then played Ninja Gaiden 2 for the first time. Beat it in one long sitting (it was easier, except the boss fights were ab it harder and the boss doesn't send you back two levels when it kills you), and then after I came back to Ninja Gaiden 1 I was able to beat it.
The 2nd final boss in Ninja Gaiden 2 is basically a slightly easier version of this guy and that's kind of how I figure dit out.

I am still convinced that the intended strategy is to die, and get sent back so you can get the spin slash and kill Jaquio in one hit. Why else would the first phase guy not respawn when you re-enter the boss room?

I've never beaten Ninja Gaiden without going back and getting the spin slash for Jaquio.

They're not flames that come up from of the bottom, they're the flames that shot towards you that changed direction because you were in the air when they passed you and thus had time to turn around. Just don't jump when he fires flames and you'll never encounter this problem.
